Case Management for Adults at Risk – Social Work services for adults at risk of abuse or neglect to include monitoring the delivery of services, and referral to appropriate service providers.
Community Alternatives Program/Children (CAP/C) - Medicaid waiver program that allows children who are nursing home level of care to remain in their homes safely.
Community Alternatives Program/Disabled Adults (CAP/DA) - Medicaid waiver program that allows disabled adults who are nursing home level of care to remain in their homes safely.
Domiciliary Care – Developmentally Disabled Adult Homes (DDA) - License and monitor family care homes and homes for the aged, ensuring safety of residents.
Guardianship and Payeeship – Court appointed guardians assure health and safety for adults. Agency serves as protective payee for those receiving social security or public assistance who are in jeopardy of or are being deprived of basic needs.
Homemaker/In-Home Aide Services – Homemakers teach money management, selection and preparation of nutritious meals, health, etc. In-Home Aides provide basic personal care and health services to homebound elderly and disabled.
Information & Referral (I&R) - Agency staff provide information to the general public about Social Service Programs and Community Services.
Placement Services – Assists the client and/or family in decision-making and adjustment regarding out-of-home placement.
Adult Protective Services (APS) - Screen, assess and evaluate reports of adult abuse, neglect and exploitation.
Adult Care Home Case Management – Provides social work services for adults who are placed in adult care homes (rest homes, family homes, etc.).
